The unanswered question in the healthcare debate — and it will never be answered, because it will never be asked — is “Who are we going to let die, and how are we going to kill them?” We routinely let the governments determine this question in war, but it’s only been in the last century or so that governments have had the resources to be able to make broad societal decisions about health care.
